Hospice Social Worker

We are looking for a part-time social worker to join our Hospice team.

Job responsibilities include:

  • Assessing patients in their homes or assisted living locations, establishing a plan of care, and consulting with the IDT team and the MD on patients progress.
  • Completing all documentation in Kinnser.
  • Adhering to the specifications of the Social Work Practice Laws, professional standards and agency policies/procedures in the performance of duties.

Qualifications:

  • LMSW or LCSW required, Masters Degree from accredited school
  • Must maintain CPR certification while employed
  • Reliable transportation, valid driver’s license, and auto insurance
  • At least 1 year experience in Hospice care services
  • Note: This part time position is paid per visit; possible full time once census grows.

Salary: $25.00 to $27.00/hour

Home Health Occupational Therapist

We are looking for 2 part-time Occupational Therapists to add to our growing team. Possible full time as needs increase.

Job responsibilities include:

  • Traveling to patient’s home or assisted living facility to complete treatment
  • Completing the OT assessment and documentation in Kinnser
  • Communicating with the Case Manager to determine plan of care
  • Communicating with the patient’s MD
  • Treating the patient based on the established plan of care
  • Coordinating with the COTA, Physical Therapist, Speech Therapist, Nurse, and other disciplines as needed
  • Providing instruction and education to patient, family, and caregivers
  • Evaluating need for interdisciplinary referral
  • Attend 2x/month case conference at the Robinson Creek office

Qualifications:

  • Bachelor’s degree from accredited college
  • Valid OT license to practice in the state of Texas
  • Must pass a background check
  • Minimum of 2 years experience in home health required

Clinical Care Coordinator

We are looking for an ambitious and energetic Home Health and Hospice Clinical Care Coordinator to help us grow. This is a full-time position.

Job responsibilities include:

  • Educating the local healthcare community on Robinson Creek Home Health & Hospice
  • Providing monthly in-service with the help of our clinical team to various offices, such as wound care, local physician groups etc.
  • Building long-term relationships with new and existing customers
  • Checking on current patients at local hospitals and rehabs

Qualifications:

  • LVN / RN required
  • Home Health or Hospice experience required
  • Proven working experience as a business development manager, sales executive or relevant sales role
  • Excellent communication skills
  • Proven sales track record
  • Proficiency in MS Office
  • Time management and planning skills
  • Experience in customer support is a plus

Salary: $50,000 – $58,000 annually + competitive bonus structure + full benefits (health insurance, dental, PTO)

Home Health Physical Therapist

We are looking to hire 2 part-time Physical Therapists to add to our growing team. Possible full time as needs increase.

RN Home Health Case Manager

Robinson Creek is looking to hire a full time office RN Case Manager to join our office team. They will work under the supervision of the Director of Nursing. This is a Monday – Friday 8:00am-4:30pm full time position.

Job responsibilities include:

  • Determine appropriateness of referrals for home health services per agency guidelines
  • Collaborate with the physicians and other health care professionals, families and clients in development of a comprehensive plan of care.
  • Provides on-call support as needed to the field staff and is part of the on-call rotation for home health.
  • Monitors documentation in the clinical chart to ensure standards are met for timeliness, accuracy and competency.
  • Will have assigned patients to case manger as determined by the Director of Nursing and other duties

Qualifications:

  • RN required
  • Home Health experience required
  • Excellent communication skills

Salary: $56,000 – $66,000 annually + competitive bonus structure + full benefits (health insurance, dental, PTO)

Field RN Home Health Case Manager

Robinson Creek is looking to hire a full time Field RN Case Manager to join our team. They will work under the supervision of the Director of Nursing. This is a Monday – Friday, full time position.

Job responsibilities include:

  • Complete Oasis assessment on all patients in their homes
  • Communicate with other team members on the patient needs
  • Collaborate with the physicians and other health care professionals, families and clients in development of a comprehensive plan of care.
  • Provides on-call support as needed and is part of the on-call rotation for home health.
  • Monitors documentation in the clinical chart to ensure standards are met for timeliness, accuracy and competency.

Qualifications:

  • RN required (3+ years nursing experience)
  • Driver’s License required
  • Home Health experience required (2+ years)
  • Excellent communication skills
  • Kinnser knowledge

Salary: $56,000 – $81,000 annually + competitive bonus structure + full benefits (health insurance, dental, PTO)
