Licensed Vocational Nurse (LVN)

We are urgently hiring for LVNs to join our Home Health team.

A Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. You will also need to attend case conferences once a week to discuss patient needs which is done virtually and/or in person.

Home Health Registered Nurse (RN)

We are looking for full-time RNs to add to our growing home healthcare team.

Job responsibilities include:

  • Reduced OASIS charting utilizing our LIVE QA program.
  • Communicate with other team members on patient needs.
  • Collaborate with physicians and other healthcare professionals, families and clients in development of a comprehensive plan of care.
  • Provide on-call support as needed and serve on the on-call rotation for home health team.

RN Case Manager

We are looking for a full-time RN Case Manager to join our office staff.

The Case Manager will work under the supervision of the Director of Nursing and will perform the following job duties:

  • Determine appropriateness of referrals for home health services per agency guidelines
  • Collaborate with physicians and other healthcare professionals, families and clients in development of a comprehensive plan of care.
  • Provide on-call support as needed to the field staff and serve as part of the on-call rotation for home health.
  • Monitor documentation in the clinical chart to ensure standards are met for timeliness, accuracy and competency.
  • Will also have assigned patients as determined by the Director of Nursing, and other duties.

Salary: $55,000 – $68,000 / year

Home Health Clinical Supervisor

We are looking for a full-time Home Health Clinical Supervisor to join our office staff.

The Home Health Clinical Supervisor will work under the guidance and supervision of the Clinical Director. This will be an office job Monday to Friday from 8am to 4:30pm. This position will oversee Case Management, field staff and clinical functions in compliance with agency policy and procedure and Texas state regulations.‪ You will direct day-to-day home health clinical operations, and all field staff. We are looking for highly organized, detail oriented individuals who possess effective communication skills both orally and in writing.

‪Basic job responsibilities will include but are not limited to the following:

  • Quality, effective communication with families/community partners, adequacy of services provided and supervision of staff through clinical knowledge, operational knowledge, and experience with regulatory compliance
  • Establishment, implementation, and evaluation of goals and objectives for care services
  • Oasis experience a must
  • Thorough oversight of the regulation/accreditation processes to ensure compliance with all state and federal guidelines
  • Coordination of care for the client, identification of all clients’ needs through the assessment process, and implementation of the plan of care and supervisory process
  • Other duties as identified by the Clinical Director
  • The Clinical Supervisor may at times provide direct care to clients

Job Requirements:

    • Registered nurse with a current TEXAS license
    • Preferred: Bachelor’s degree in Nursing

    • Minimum two years of direct employee supervision
    • Demonstrated ability to supervise and direct staff
    • Possess exceptional nursing assessment skills
    • Excellent interpersonal skills
    • Analytical, critical thinker
    • Excellent customer service skills and ability to deal tactfully with clients, staff and community
    • Case management skills are a must
    • Must understand regulatory/quality compliance with Home Health
    • Kinnser knowledge a must
